Alternatively, you can use our online booking system on our website or visit any of our locations in person to schedule an appointment with one of our healthcare providers.

For your first appointment, please bring:
  • A valid government-issued photo ID
  • Your insurance card (if applicable)
  • List of current medications and supplements
  • Medical records from previous providers (if available)
  • Any relevant medical imaging or test results
  • A list of questions or concerns you'd like to discuss

Insurance & Payment

Healthy Medical Guide accepts most major insurance plans including Medicare and Medicaid. We work with Aetna, Blue Cross Blue Shield, Cigna, UnitedHealthcare, and many other providers. Services & Treatments

Our healthcare providers will work with you to determine the most appropriate service or treatment based on your specific health needs, medical history, and goals. We begin with a comprehensive consultation and assessment to understand your condition and recommend the best course of action. You can also schedule a free 15-minute phone consultation with one of our care coordinators to discuss your options before booking a full appointment.

While we offer urgent care services for non-life-threatening conditions during business hours, we are not equipped as an emergency room. For life-threatening emergencies such as chest pain, severe bleeding, difficulty breathing, or stroke symptoms, please call 911 immediately or go to the nearest emergency room. After emergency treatment, we can provide follow-up care and coordinate with the hospital physicians.

Yes, all our physicians are board-certified in their respective specialties. Our healthcare team includes licensed and certified nurse practitioners, physician assistants, nurses, and technologists who meet rigorous standards for education, training, and experience. We maintain ongoing continuing education to stay current with the latest advancements in medical care.

Technical & Privacy

You can access your medical records through our secure patient portal available on our website. Once registered, you can view test results, appointment history, and medical records. If you need physical copies, you can submit a records release form at any of our locations. There may be a fee for printing extensive medical records as allowed by state law.

We take patient privacy seriously and comply with all HIPAA regulations. Your health information is stored securely in our electronic health records system with multiple layers of protection. We limit access to your information to only those healthcare providers directly involved in your care. We understand that sometimes schedule changes are necessary. Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment. This allows us to offer that time slot to another patient. You can cancel or reschedule through our patient portal, by calling our office, or by responding to your appointment reminder text or email. Late cancellations (less than 24 hours notice) may be subject to a fee.
